2/5/2024 EnchantaVaughn Approved QMHPA arrived to the home of Ms. Broadnax. Upon arrival to the home, Ms. Broadnax appeared to be in a positive mood evident by her facial expressions such as a smile on the face. Ms. Broadnax did not present any symptoms of S/I or H/I at the time of the session. QMHPA observed that Ms. Broadnax displayed a stressful state that related to her sister calling her and asking her to come assist with household chores in her home. QMHPA guided Ms. Broadnax in interacting in a stress management exercise that consisted of closing her eyes, taking slow breathes in and out; and saying to herself (out loud) positive affirmations. QMHPA and Ms. Broadnax engaged in this exercise for 15 minutes. QMHPA used slow Jazz sounding music as a back drop for the exercise. QMHPA assisted Ms. Broadnax through modeled behaviors in discerning between relaxation and stress. QMHPA utilized modeled behaviors to assist Ms. Broadnax in understanding how to apply the methods to her daily life. QMHPA moved onto assist Ms. Broadnax in recognizing and understanding how her impulsive behaviors caused her the most issues in functioning and responding to others.QMHPA explained to Ms. Broadnax that she was not to over-react or respond in a negative manner to the response of the person, and to choose someone she trusted and that would painfully honest with her. Ms. Broadnax and QMHPA engaged in general conversation in regards to Ms. Broadnax’s frustrations with the people she asked for help mistreating her and then they ask her n return to assist them with a task. Ms. Broadnax shared that she had not been sleeping well. Ms. Broadnax actively participated in the session evident by her responses and questions. Ms. Broadnax expressed to QMHPA that she is tired of asking others to assist her with certain task within her home but she is always there to lend the helping hand to her family members. Ms. Broadnax stated to QMHPA that she loves to listen to her music to help calm her nerves when she is upset with family issues that she cannot solve.
